かれがたになりける人に、すゑもみぢたるえだにつけてつかはしける

よみひとしらず

今はてふ　心つくはの　山見れば　こずゑよりこそ　色かはりけれ

Translation

To a person who was staying away, she sent this attached to a branch that was changing color at the tips.

This is a pivot on 心づく, indicating the speaker understands the relationship is over, and つくばの山, a place name associated with autumn leaves.「つくば山に紅葉よめり」（聞書）

こずゑ

This plays on 来ず. Just as the color is changing starting from the 梢 on the branch, the man's feeling is changing starting from the 来ず (not visiting).
